Instructions for Grantmakers

This sample Application Assessment Form can be adapted to suit your own program’s needs.

This form is part of a two-form set of template SmartyGrants application assessment forms. This form is most likely to be useful for an internal assessor (usually carried out by staff). It focuses on eligibility requirements. The second form, also available through SmartyGrants, would be most useful for an external grants assessment team. It focuses on likelihood of creating change and alignment with the program's outcome goals.

Note that conditional logic has been applied to this form. If you remove sections/ questions, this logic may be affected. Use 'Preview' mode to view where logic has been applied, or click on 'Section' or 'Page' headers to see conditions on the left-hand panel.

Some questions also have validation applied (e.g. compulsory fields).

By default this form has been set to 'private'. If you wish to share this form with external assessors so they can view the responses, you can change the form access in the form settings area (learn more about this setting on the Help Hub).

Any assessment of an application should be based on an organisation's own criteria for funding. You should therefore delete, add, edit and adapt the questions (including hints, validation and conditional logic) to suit your own program's priorities and objectives. Delete what you don't need – as a rule of thumb, don't ask a question if you don't know if or how you will use the answers.
