Collection of personal information

We are collecting your personal information to verify your eligibility under the Supply Chain Capacity Program, and if approved, provide the recommended support.

Including: 

  • your name and contact details;
  • payment information (including your bank details);
  • details about your business (including ABN, trading name, type of entity, GST registration etc);
  • trust details and beneficiaries;
  • business financial statements
  • details of co-contributor (cash and/or in-kind) to your proposal;

Disclosure of personal information

We may share this information with subject matter experts assessing applications, independent financial assessors, evaluation contractors and other government agencies such as the Department of Water and Environmental Regulation and Regional Development Commissions involved in risk assessments, in order to assess, verify and evaluate your application under this scheme of assistance and any other government agencies as required by law.

Your Application - Information

Before completing this application form, you should have read the Supply Chain Capacity Program Guidelines 

Please read each section and complete each question as requested. It is recommended that you allow yourself enough time to review the application and submit it well before the due date. The closing date for your detailed application is 5:00pm (AWST) on Friday 24 April 2026.

LATE APPLICATIONS CANNOT BE ACCEPTED.

Please note, supporting documents are supplementary to your application. Please ensure you answer all the questions and provide relevant details where required. 

You must submit a complete application, including all mandatory fields and required supporting documentation. Incomplete applications may be deemed ineligible and not be assessed.

Applications seeking in excess of $500,000 are expected to provide more detailed information in the application and/or attachments.

Confirmation of Eligibility

Please refer to 'Applicant Eligibility Requirements' in the Guidelines to confirm your eligibility before completing this application. Eligible applicants must meet ALL of the following: 

Is the applicant operating or intending to operate in WA’s sheep supply chain? * Required
Is the applicant a private sector business (including incorporated trustees), a public company, a grower cooperative, a conglomerate, a formal/informal partnership or similar organisations having an Australian Business Number (ABN) and registered for GST in Australia? * Required
Does the applicant have an account with an Australian financial institution (eg. bank, credit union, building society)? * Required
Is the applicant authorised to enter into a legally binding agreement with the WA Government? * Required
Has the applicant been in operation for a minimum of two years (or similar established track record or demonstrated record of industry operation)? * Required
Is the applicant financially viable, with the intention to continue operating into the foreseeable future and there are no plans, events and/or circumstances known to the applicant which would result in their business ceasing to trade? * Required
Can you confirm the applicant is not under any formal compliance or legal actions? * Required
Does the applicant have the financial capacity to fund the minimum cash contribution (see guidelines for more information)? * Required
Does the applicant intend on delivering their project by 30 June 2028? * Required
Does the applicant intend to locate their project in regional and/or metropolitan WA? * Required
